docs: #345 A/B complete — pre-existing, Campaign S exonerated wholesale

The pre-campaign binary shows the identical 0.0%-yield signature across
eleven consecutive sustained-input windows at the same slope with the
same protocol. Older defect, newly noticed. The throwaway A/B worktree
is removed.

(the UP-normal zero-displacement step-down-refusal shape leads); theory work
remains forbidden until the mechanism session instruments it.
