Clean up /mm help commands

- Remove dead no-op commands (harmonyraw, debugtaper)
- Remove duplicate command (initgui, same as gui)
- Hide debug/test commands from help output (vtanktest, decalstatus,
  decaldebug, testprismatic, testdeath, testtaper, debugupdate)
- Clean up descriptions for consistency
