feat: increase portal detection range from 60m to 240m
60m was too narrow (~half a house). 240m matches the typical in-game view distance for terrain features (1 AC coordinate unit = 240 game units). Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>
